For many organisations forms demonstrate a cost effective way to gather data from a large number of potential respondents. In some cases they are the only practical way to reach a group of people large enough to allow the statistical analysis of results. Therefore a well-designed form or questionnaire that is used correctly can gather important information that can can be used by you to measure not only performance and satisfaction but many different types of criteria.
